Why Pharmacology Matters in Wound Care

Wound care is often viewed through the lens of dressings, debridement, advanced therapies, and procedural interventions. However, pharmacology underpins many of the physiologic processes that determine whether a wound heals, stalls, or deteriorates. Every wound care professional encounters patients whose medications directly or indirectly influence inflammation, tissue repair, infection risk, circulation, pain, and overall healing capacity.

The importance of pharmacology in wound care continues to grow as the patient population becomes older, more medically complex, and more likely to be treated with multiple medications. Understanding how medications affect wound healing is no longer a specialized skill—it is a foundational component of comprehensive wound management.

The Growing Burden of Chronic Wounds

The need for pharmacologic knowledge is closely tied to the expanding burden of chronic wounds. Chronic wounds affect millions of individuals worldwide and represent a significant challenge for healthcare systems. In the United States, Medicare beneficiaries with wounds account for billions of dollars in annual healthcare expenditures, with chronic ulcers, diabetic foot ulcers (DFUs), pressure injuries, and venous leg ulcers among the most costly conditions.1

Diabetes alone affects approximately 38.4 million Americans, representing 11.6% of the U.S. population.2 The lifetime risk of developing a diabetic foot ulcer has been estimated to range from 19% to 34%, making wound complications a common consequence of this chronic disease.3 Meanwhile, pressure injuries affect more than 2.5 million patients annually in the United States and remain associated with substantial morbidity, mortality, and healthcare costs.4

These populations frequently receive medications for diabetes, cardiovascular disease, autoimmune disorders, cancer, pain management, and infection prevention. Consequently, pharmacology becomes inseparable from wound care practice.

Polypharmacy is the New Normal

One of the strongest reasons wound care professionals need pharmacology expertise is the prevalence of polypharmacy.

According to data from the Centers for Disease Control and Prevention (CDC), nearly 90% of adults aged 65 years and older take at least one prescription medication, while approximately 54% take four or more prescription drugs.5 Patients with chronic wounds often exceed these averages because they commonly have multiple comorbid conditions, including diabetes, peripheral arterial disease, chronic kidney disease, obesity, and cardiovascular disease.

Polypharmacy introduces numerous clinical considerations. Medication interactions, cumulative adverse effects, altered drug metabolism, and adherence challenges may all influence healing trajectories. Research has shown that polypharmacy is associated with increased risks of hospitalization, adverse drug events, frailty, and mortality among older adults.6

For wound care clinicians, this means that understanding medications extends beyond knowing drug names; it requires appreciating how complex medication regimens may affect overall patient health and wound-related outcomes.

Medications Can Influence Every Phase of Healing

The wound healing process depends on tightly regulated inflammatory, proliferative, and remodeling phases. Numerous medications have the potential to influence these biologic processes.

For example, medications that alter immune function, inflammatory signaling, coagulation pathways, or cellular proliferation may affect tissue repair dynamics. Likewise, drugs used to manage chronic conditions can influence vascular function, glucose control, oxygen delivery, and nutritional status—factors known to affect healing outcomes.

The impact of medications on wound healing has become increasingly important as biologic therapies, immunomodulators, targeted cancer treatments, and newer metabolic agents enter widespread clinical use. The expanding pharmaceutical landscape means clinicians are encountering a broader range of medication effects than ever before.

A foundational understanding of pharmacology provides the context necessary to recognize how medications interact with the biologic processes that support wound closure and tissue regeneration.

Infection Management and Antimicrobial Stewardship

Pharmacology also plays a central role in one of wound care's most persistent challenges: infection.

Antimicrobial resistance has emerged as a major global public health threat. The CDC estimates that more than 2.8 million antimicrobial-resistant infections occur annually in the United States, resulting in over 35,000 deaths.7 Chronic wounds frequently harbor complex microbial communities, making infection prevention and management particularly relevant.

At the same time, inappropriate antibiotic use remains a concern across healthcare settings. The Centers for Disease Control and Prevention reports that approximately 30% of outpatient antibiotic prescriptions may be unnecessary.8

As healthcare systems increasingly prioritize antimicrobial stewardship, wound care professionals must understand the pharmacologic principles underlying antimicrobial therapy, resistance development, adverse effects, and medication safety. Pharmacology knowledge provides the foundation necessary to participate effectively in these broader stewardship initiatives.

Chronic Disease Pharmacotherapy Shapes Wound Outcomes

Many of the diseases most closely associated with chronic wounds are managed primarily through medications.

Diabetes, cardiovascular disease, autoimmune disorders, renal disease, and peripheral arterial disease all rely heavily on pharmacologic interventions. Because these conditions directly influence wound risk and healing potential, medication management becomes a critical component of understanding the overall patient picture.

For example, diabetes remains the leading cause of nontraumatic lower-extremity amputation in the United States.2 Glycemic control, cardiovascular risk reduction, and management of diabetes-related complications are largely achieved through pharmacotherapy. Similarly, medications used to treat cardiovascular disease affect circulation, thrombosis risk, and tissue perfusion.

As new drug classes continue to emerge, wound care professionals increasingly need a working knowledge of the pharmacologic therapies shaping patient outcomes across multiple disease states.

Patient Safety Depends on Medication Awareness

Medication-related harm remains a significant healthcare concern.

The World Health Organization estimates that medication errors and unsafe medication practices cause approximately one death every day and injure millions of patients annually worldwide.9 Older adults and medically complex patients are particularly vulnerable because of high medication burdens and multiple chronic conditions.

Patients receiving wound care often transition between hospitals, skilled nursing facilities, outpatient clinics, home health services, and primary care practices. Each transition increases the risk of medication discrepancies, communication failures, and adverse drug events.

Understanding pharmacology helps clinicians identify potential medication-related risks, recognize adverse effects that may affect wound outcomes, and contribute to safer interdisciplinary care.

Why Pharmacology Education is Increasingly Important

Despite its relevance, pharmacology is sometimes viewed as peripheral to wound management rather than central to it. Yet virtually every wound care encounter involves medications that influence healing, symptom management, infection risk, or chronic disease control.

The rapid growth of specialty pharmaceuticals, biologic therapies, antimicrobial stewardship initiatives, and complex chronic disease management has elevated the importance of pharmacology knowledge across all healthcare disciplines. For wound care professionals, this knowledge provides a framework for understanding the broader physiologic and therapeutic factors affecting patient outcomes.

A strong foundation in pharmacology supports more informed clinical decision-making, strengthens interdisciplinary communication, and enhances appreciation for the many medication-related variables that influence healing.

As wound care continues to evolve, pharmacology is no longer simply a supporting topic—it is a core competency that affects virtually every aspect of patient care.
